It is important that we know if you are a carer for one of our patients. If we know that you are a carer we can offer you more support, register you with Guideposts who will contact you to discuss how your needs and the patients needs may be met.
Please ask at the desk (or complete the form below) to register as a carer, we will ask you to complete the necessary forms and ensure your records are updated. Carers are not given patient information or do we discuss patients without the patients written authority.
