Fair processing of patient information

To give you the care you need, we keep information about your visits to the surgery. This information contains your personal details such as name, date of birth and address. We sometimes share your personal information to other healthcare organisations such as when your GP refers you to the hospital. We also use your personal information for administrative tasks such as inviting you for health screening.

Sometimes the Law requires us to pass on information for example we have to report births, deaths and certain diseases. The Law sets out how your personal information is used and you have the right to see the information we hold about you.

We also need to use your information to improve our services and check that they are up to standard but we do not share information that could identify you. All NHS staff have a legal duty to keep information about you confidential and they follow a staff code of practice on protecting patient confidentiality.

We do not pass any information about you to any other organisations without your written consent.
