When registering with the surgery, you may be asked to provide Proof of Identity as part of your registration process. Please see the list below for accepted documents. A mobile photo of the document is acceptable if responding through our text link, as long as it is readable. If you are unable to do this, bringing a paper copy to the surgery front desk for verification is also acceptable.

- Credit card bill or statement
- Bank statement
- Bank reference letter
- Mortgage statement or contract
- Letter issued by a public authority (e.g. a courthouse)
- Company payslip
- Car or home insurance policy
- Car registration
- Authorized change of address form
