Proxy access on behalf of Children and Young People

Please note the below information before requesting Proxy Access for your child/children.

Any request made by parents for proxy access to online accounts will be thoroughly checked and verified by our registrations team, before being sent for GP review to ensure access is appropriate and necessary.

Upon the child’s 11th birthday, our GP computer systems will automatically restrict the scope of existing proxy access. Parental proxy access may be reinstated if, after discussion with the parent(s) requesting access, the child’s GP believes that proxy access would be in the child’s best interest, or is required for patients continued care in the case of lack of competency. Once you receive a text informing you of the impending restriction of proxy access for a child turning 11, a “11-16 proxy access request” form can be collected at the surgery front desk. This will then be forwarded to one of our GP partners, who can make a decision on continuing to grant access for Children aged 11-16.

For children younger than 11 years old, Proxy access is granted as parental responsibility, subject to a GP’s confirmation that access is appropriate and in the best interests of the child.

From 11–16, a parent with proxy access will be able to manage certain elements of the young person’s record, such as demographic data, make appointments and order repeat prescriptions, but they will not be able to see the young person’s past appointments or clinical record.

Upon the young persons’ 16th birthday, the systems will switch off all the remaining proxy access except where the young person is competent and has given explicit consent to the access, or is deemed not competent and requires parental/guardian management of their continued care.

Any request for parental proxy access for children under 11 must be in written format using our online services form and can be sent to our practice email cwicb.oldmillsurgery@nhs.net or handed in to the front desk, if required the proxy access requester must be able to provide proof of identity if asked for by our Administration team. This is to ensure the safety of our patients and their data. The online services form can also be collected from our front desk, or printed from the forms section of our website. For proxy access on behalf of children that are 11-16 years old, we require our “11-16 Proxy access form” to be filled out and handed in to front desk (this form is only available at the surgery, and cannot be printed from our website). Identification of the proxy requester will be required by our front desk team upon this form being handed in. This will then be checked by our GP team to ensure access is appropriate, and confirmation sent to the proxy requester.

Proxy access can be restricted/deactivated by our Clinical team, Admin team or system providers at any time if they believe account access is being abused, or infringes on the care or rights of the account holder/record holder.
